For tracking changes, I always keep a modification history section, right in the project's source code. Any changes I make to a version is documented there, and the date that was done is shown too. It is alot of over-head work for me (along with the rest of the internal documentation I do for my projects), and I don't always remember to actually update the logs, but it works out in the long run.
